1.) Are you pregnant, nursing, or planning on getting pregnant or nursing in the next 6 months?
2.) Do you have a history of heart attack in the last 2 weeks?
3.) Do you have a history of arrhythmias or irregular heartbeats?
4.) Do you have a history of unstable angina or report chest pain with strenuous activity?
5.) Do you have a history of mental health disorder and are perceived to be unstable?
